Record one failure mode per row and link it to a component in the facility model, with effects, causes, Severity × Occurrence × Detectability ratings and corrective measures.

What you will do

  1. Select the relevant component from the facility model.
  2. Describe the failure mode, its effects and causes.
  3. Rate S/O/D before and after corrective measures. The RPN is recalculated automatically.

Pitfall to avoid. Failure modes require an existing facility model. Complete that page first if it has not already been done.
